The Division of Transplant, specializing in Lung is actively searching for an Advanced Practice Provider to join their team.
The Transplant APP is an advanced practice nurse who endeavors to provide and maximize the quality of care to patients referred to the Penn Transplant Program with end-stage organ failure.
The Transplant APP provides direction in the management of patient care throughout all phases of the transplant process: organ failure, evaluation, transplantation, and follow-up care.
The Transplant APP performs a systematic assessment, evaluation and develops a plan of care for patients and their significant others; develops a written, continuous record for each patient; implements identified medical and nursing interventions; and evaluates outcomes.
The Transplant APP serves as an integral member of the transplant team in meeting the needs of the transplant patient and ensuring continuity of care throughout the transplant process.
Minimum Qualifications: Licensed to practice professional nursing in the Commonwealth of Pennsylvania, B.S.N & M.S.N., and Board Certified as an Acute Care Nurse Practitioner. OR Masters Prepared Physician Assistant with active PA licensure.
Prior to employment, the CRNP/PA must be reviewed by the Credentials Committee and approved by the Medical Board.
Required to have four or more years’ experience as a professional nurse or PA.
