Summary:

  • The Social Worker provides and documents individualized social work assessment, care planning, intervention and outcome evaluation for patients in the ambulatory practice setting.

Responsibilities:

  • Works within the scope of practice as designated by the individual’s state board of Social Work
  • Demonstrates competence in the assessment, treatment and care within age specific guidelines
  • Utilizes office guidelines for telephone triage. Speaks with provider if call is outside their scope of expertise
  • Interview patients and relatives to obtain social history and assist patients with maximum benefits from medical care
  • Obtains patient medical coverage in order for the patient to receive the maximum services from insurance companies
  • Communicates observations, facts and comments to physician or requesting agency
  • Assists in the case management of patient health services delivery
  • Coordinates transportation and special care services
  • Documents patient observation in patient medical charts per practice protocol
  • Assist and facilitates completion of medical forms requested by agencies
  • Establishes positive relationships with hospital departments (e.g. central registration, pre-admission testing, hospital social services, etc) and outside agencies to ensure care and services for patients
  • Explores patient home situation and need for community services/home Health referrals
  • Assesses patient financial circumstances in order to pay for medical services

Credentials:

    Education or Equivalent Experience:

    • Bachelor of Arts or Science (Required)
    • social work or social services; major concentration in health care or social work field, such as counseling, teaching, social work, psychology 0-1 years Prior clinical experience with social work services in acute care facility; Prior coordination and facilitation of Medicaid/Medicare support programs and counseling services 1+ years Related outpatient experience preferred
