- Provides Behavioral Health Assessments and Case Management Services to Homeless Adults and their Children at designated shelter sites in the Philadelphia Shelter System. The Shelter Outreach Specialist coordinates the connection of participants to behavioral health outpatient services, substance abuse services and linkage to other community supports. Will also be responsible to coordinate and participate in a Psychiatric Medication Clinic for identified homeless individuals. Has experience working with the homeless population. Should have experience accessing the needs of people with a behavioral health diagnosis. Needs to be flexible and adaptive to changing work environments and requirements. Should have a working knowledge of the local shelter, housing, medical and behavioral health resources for homeless individuals and families in the City of Philadelphia.
Education or Equivalent Experience: Bachelor’s degree from an accredited program in an area of human services, which includes, but may not be limited to the following: psychology, sociology, political science, counseling, criminal justice, pastoral counseling, elementary/special education, or family therapy.
