Summary:
- Assigns ICD-10-CM, ICD-10-PCS and CPT-4 codes to patient records based upon physician documentation. Abstracts discharge data from medical records for the purpose of processing patient bills and compiling statistical data including diagnostic, procedural and physician indices. (PMPH HIM Clinical Documentation)
Responsibilities:
- Coding: Inpatient - Assigns ICD-10-CM and ICD-10-PCS diagnosis and procedure codes to Hospital and/or Princeton House medical records while maintaining a 95-100% accuracy rate according to Department guidelines on a daily basis. Coding: Outpatient - Assigns ICD-10-CM and ICD-10-PCS diagnosis and procedure codes to Same Day Procedure, Emergency, Endoscopy, and other Special Procedure medical records while maintaining a 95-100% accuracy rate according to Department guidelines on a daily basis. Coding: Outpatient - Assigns CPT-4 procedure codes to Same Day Procedure, Emergency Department, Endoscopy, and Special Procedure medical records while maintaining a 95-100% accuracy rate, on a daily basis. Coding: Inpatient - Abstracts Hospital and/or Princeton House medical records according to Department procedures while maintaining 95-100% accuracy on a daily basis. Coding: Outpatient - Abstracts Same Day Procedure, Emergency, Endoscopy, and other Special Procedure medical records according to Department procedures while maintaining 95-100% accuracy on a daily basis.
Credentials:
- CCS or RHIA or RHIT (Required)
Education or Equivalent Experience: High School Diploma Required.
● Strong background in medical terminology, disease process, ICD-10-CM, ICD-10-PCS, CPT-4 coding required.
● Knowledge in encoder and abstracting systems required.
● Knowledge of DRG and APC reimbursement methodologies required.
