Summary:
- Under supervision from the Director of Behavioral Health Services and/or the Program Manager, the Opioid Use Disorder (OUD) Care Team Coordinator will be responsible for coordinating the enhanced care management services for individuals enrolled in the Center of Excellence program.
Responsibilities:
- Coordination of the care team members, to include Certified Recovery Specialists (CRS), care coordinators (CC), and other members of the care team as indicated.
- Coordinate the Medication Assisted Treatment (MAT) in the outpatient clinic and/or Penn’s Primacy Care Practices.
- Monitor the documentation of each care team members.
- Provide individual therapy to members as assigned.
- Ensure the overall coordination and collaboration of care team members and coordinate the daily affairs of the team’s logistical needs in event planning, meetings and team communication.
- May serve as a public relations representative to communicate on behalf of the team to the general public.
- May also assist the Program Manager by fulfilling any needed requirement the Manager needs.
Credentials:
- National Certified Addictions Counselor
- Certification in Addictions Counseling preferred
- Licensed Clinical SW
- License in Social Work, Professional Counseling, or other independent licensure (LCSW, LPC, PhD, etc.) preferred
Education or Equivalent Experience:
- Master of Arts or Science (Required)
- Psychology or a related field
2+ years Two years’ experience in mental health/addictions field and treatment of the program’s patient population
