Position Description:

The Transplant Coordinator II is a professional nurse who safely practices with the UPHS professional practice model and demonstrates UPHS core values that reflect its mission. The Transplant Coordinator II provides direct care and coordinates care for patients with end-stage organ failure referred to the Transplant Program. The patients range in age from young adult to elderly. In this role, you will provide direction in the management of patients throughout all phases of the transplant process, perform a systematic assessment, evaluation, and develop a plan of care for the patient and their significant others. You will also serve as an integral member of the transplant team in meeting the needs of the transplant patient and ensuring continuity of care throughout the transplant process.

Minimum Requirements:

The qualified candidate must possess current licensure as an RN in the Commonwealth of PA and 4 years of professional nursing experience. 2 years of experience in the transplant specialty is preferred.

Additional Information: Work hours are 8:30-5, M-F, with some rotational on-call duties. Work schedule is remote and in-person hybrid format.
