Summary:

  • Responsible for reviewing all Face to Face documentation for clinical accuracy/completeness for Penn Medicine at Home. Documentation may be in EMR or on a F2F form. He/She will work with orders management team, Face to Face team and Central Intake to verify that provider information is correct. He/She will utilize, Medview, EPIC and HomeWorks in order to gather appropriate data to provide to physician and/or physician agent, or to satisfy Medicare regulatory compliance. He/She is responsible for helping track outstanding physician documents according to department procedures. Responsible for immediate notification to manager for any issues related to incorrect physician identification or physician refusal to sign. Tracking and reporting compliance in accordance with the policies and procedures of PMAH.

Responsibilities:

  • Promotes a good relationship with physicians, physician offices, in order to facilitate form/documentation completion, signature and return.
  • Adds the Face to Face template to the Plan of Care and accurately completes the required documentation.
  • Supports the Penn Patient Experience program by utilizing scripted responses and tools to promote and maintain a responsive, knowledgeable and accessible approach to all internal & external customer contacts.
  • Works with Face to Face team to resolve any outstanding documentation issues in order to satisfy Medicare regulations.
  • Tracks documents daily according to department process.
  • Adheres to agency productivity standards.

Credentials:

  • Licensed Practical Nurse (Required)

Education or Equivalent Experience:

  • Other (Required)
  • NLN accredited licensed practical nursing program 3+ years licensed practical nurse and experience in chart review
  • Bachelor of Science Nursing
  • If a Registered Nurse, must have a BSN from an accredited school of nursing 2+ years Experience as a Registered Nurse and experience in chart review
