Summary: The Outreach Workers participate in Philadelphia’s Chronic Homeless Initiative (CHI) to provide general community outreach, focused engagement and case management for homeless adults with severe and persistent mental health issues and co-occurring disorders. Conducting outreach to the homeless with a SEPTA Officer during the entire shift. 

Travel: Walking and using Septa for travel.

Education or Equivalent Experience: Bachelor degree in mental health / social services related field, no experience at entry level; or non-degreed with two years mental health / outreach experience.
