Summary:

  • The Admission RN is responsible for the entry of patients and families into the spectrum of services provided by Penn Medicine Hospice. As a member of the Interdisciplinary Team (IDT), the Admission RN provides direct care to adult patients and demonstrates safe practice within the domains of clinical judgment and decision-making, nurse-patient/family relationship, clinical leadership, and clinical scholarship. The Admission RN works closely with others on the healthcare team to facilitate the coordination of care across the continuum. The individual reviews documentation of potential clients who are referred to hospice, arranges for initial contact in a timely fashion and communicates plans for contact with client and family and referral sources. In addition, the Admission RN provides information to potential hospice patients, assists with the decision making process, performs assessments that assure the appropriate level of care is delivered, and expedites entry into service delivery systems.
  • The Admission RN nurse must attend to the values and rituals important to the patient-family-community environments as well as upholding nursing practice and safety standards. The Admission RN instructs the primary caregiver of immediate care needed to be provided and informs primary nurse of the initiation of the plan of care. In addition, the Admission RN is responsible for practicing within the Hospice regulations and reimbursement criteria. As such, the nurse is responsible for determining the skilled nursing care and visit pattern to maintain quality and meet the patient and family's goals. The Admission RN will have a working knowledge and understanding of hospice, the interdisciplinary team concept and their individual role. The Admission RN functions within the interdisciplinary team to facilitate communication to promote and support symptom and disease management.

  • Professional Practice Competency Domains Definitions: Continuous Quality Improvement (CQI): The nurse utilizes data and QI methods to identify potential and actual problems and
  • opportunities to provide care that is safe, timely, efficient, effective, and equitable.
  • Evidence Based Practice and Research: The nurse evaluates and integrates best current evidence with clinical expertise and patient and family preferences and values for the delivery of optimal health care and system effectiveness.
  • Leadership: The nurse effectively collaborates and applies innovative, systems thinking to engage in systematic, evidence based problem solving and decision making to promote effective changes within a complex care delivery system supporting the vision of Penn Medicine.
  • Person and Family Centered Care: The nurse recognizes the patient (or the patient’s designee) as the source of control and a full partner in providing compassionate and coordinated care based on respect for the patient’s preferences, values, and needs.
  • Professionalism: The nurse demonstrates a commitment to the nursing profession through lifelong learning, adherence to ANA’s Code of Ethics for Nurses, participation in a professional organization and advancing community outreach.
  • Safety: The nurse minimizes risk of harm to patients, families, providers and self through system effectiveness and individual performance.
  • Technology/Informatics: The nurse utilizes appropriate information and technology to communicate, manage knowledge, mitigate error, and support decision making across the continuum.
  • Teamwork: The nurse effectively engages in the process of cooperation, coordination, and collaboration in an effort to provide for safe, quality outcomes for patients within inter and intra-professional teams, including virtual teams.

Credentials:

  • Basic Cardiac Life Support (Required)
  • BLS/CPR, as a healthcare provider as per the American Heart Association
  • Car Insurance (Required)
  • Driver's License (Required)
  • Registered Nurse - PA (Required)
  • PA, NJ, and DE RN licensure depending on geographic location and scope of responsibilities Certification in a National Nursing Specialty, appropriate to practice needs or patient population

Education or Equivalent Experience:

  • Bachelor of Science Nursing (Required)
  • BSN from an accredited school of nursing and more than 15 months relevant professional nursing experience and completion of the Nurse Residency Program, if applicable Infusion experience preferred
