General Summary Statement

Provides comprehensive, quality nursing care through application of the nursing process.

Adheres to Nursing Policies and Procedures and Organizational Goals and Objectives.

Essential Duties and Responsibilities

Patient Care Responsibilities:

  1. Provides age appropriate care for the population served. Gives consideration to growth & developmental phase of life, to age related factors such as thermal regulation, nutrition, medication administration, nutrition, safety and functional needs of the patient.
  2. Seeks help from RN when needed.
  3. Uses appropriate resources for patients with special needs such as language barriers, sensory problems, altered cognition, cultural and religious needs.
  4. Integrates principles of infection control & infection prevention into practice. Utilizes eyewear, gloves and gowns during procedures and specimen collection (urine etc).
  5. Assists physician and nurse practitioners with procedures.
  6. Performs vital signs, urine dip, and weight per policy.
  7. Labels specimens as directed for transport to lab. Completes lab request for specimen.
  8. Cleans dirty instruments and takes to Central Sterilizing per policy.
  9. Checks for outdates monthly.
  10. Maintains refrigerator log.
  11. Performs QC on Uristix daily.
  12. Maintains lab log and checks monthly for results received. Retrieves any missing results from lab.
  13. Inventories, orders and restocks supplies in exam rooms.
  14. Uses resources and references for reviewing policies and procedures, information needed to complete job duties.
  15. Maintains patient safety.
  16. Demonstrates competency in assessment skills. Performs pain assessments and intervenes to optimally meet patient’s needs for comfort.
  17. Identifies situations that warrant patient rescue and takes appropriate action, e.g. Rapid Response or Code Blue/Pink.
  18. Adheres to the 5 rights of medication administration per policy and procedure.
  19. Administers and interprets TST. Administers IM injections as needed.
  20. Integrates principles of infection control & infection prevention into practice.
  21. Adheres to aseptic technique as appropriate.
  22. Uses resources and references for reviewing policies and procedures, medications, and clinical information. Seeks clarification when needed from RN, Manager or MD.
  23. Prioritizes and organizes work effectively within assigned shift.
  24. Maintains patient safety through teaching patient about fetal movement, kick counts, reinforcing diet and medication instruction, self care, warning signs of complications of pregnancy.
  25. Cleans equipment.

Communication

  1. Communicates patient and unit information respectfully to the RN and interdisciplinary care team in a timely manner.
  2. Informs the RN or Manager when experiencing difficulty completing an assignment.
  3. Uses clear effective communication.
  4. Accepts changes in assignments.
  5. Demonstrates a cheerful, friendly manner while on duty.
  6. Informs RN when leaving the unit for any reason.
  7. Accepts assignments and performs as a team member.
  8. Organizes information into a meaningful format.
  9. Introduces self to patient and explains procedures to patient and family.
  10. Utilizes the Language Line.
  11. Utilizes principles of teamwork to provide best outcome for patients.

Documentation

  1. Demonstrates writing skills clearly that are appropriate for healthcare communication.
  2. Utilizes only approved medical abbreviations.
  3. Documents patient care in an accurate, professional and timely manner utilizing documentation forms according to policy and procedure.
  4. Documents patient care that is relevant to patient’s diagnosis, problems and reflects patient’s status.
  5. Obtains results for Soarian, CDC labs, Quest, Lab Corp, Interdoc, etc.

Professional Role Development

  1. Develops own nursing knowledge via practice, observation, professional literature, networking and attending continuing education programs.
  2. Promotes best practice by demonstrating a commitment to quality patient care and the profession of nursing.
  3. Promotes innovation and creativity in the practice environment.
  4. Accepts constructive feedback about performance or behaviors and shows evidence of change.
  5. Adheres to the Department of Nursing Dress Code and maintains a professional appearance according to policy standards
  6. Maintains confidentiality.
  7. Attends staff meetings.
  8. Performs as a team player.
  9. Maintains good attendance/lateness standards in accordance with Hospital policy.

Department of Nursing and Organization Support

  1. Attends assigned committee meetings and contributes to the committees.
  2. Communicates support of organizational values and strategies.
  3. Maintains good attendance/lateness standards in accordance with Hospital policy.

Departmental Support

  1. Maintains equipment in working order. Uses equipment appropriately, follows Department policies for equipment and supply use and maintenance.
  2. Performs assigned duties to maintain unit in a state of survey readiness and maintain quality initiatives. For example, quality control measures, emergency equipment checks, medication preparation area cleanliness, etc.
  3. Attends staff meetings on a regular basis.
  4. Keeps patient care area and work environment neat and organized.

Education Training and Experience

Graduate of an accredited LPN Program.

Experience preferred, however training and specialty specific education programs are provided during orientation, and must be successfully completed as scheduled.

Basic computer skills: keyboard, mouse use. Ability to complete data entry on clinical documentation screens. Ability to use email and internet. Ability to successfully complete on the job training for CCH computer programs.

Ability to speak, read, write and understand English.

Must be bilingual in Spanish. 

Maintains CPR and PPD within CCH deadlines.

Licensure/Certification

Current Pennsylvania LPN License.

CPR certification required.
