Penn Medicine Princeton House Behavioral Health is searching for a full time Discharge Planner for our Princeton location.
Summary:
- Implements discharge plans which have been approved by the Clinical Treatment Team. Provides assistance with applications to Nursing, Group and Boarding Homes, makes contact with community agencies and sets up follow-up appointments prior to discharge. Establishes relationships with outside agencies needed for discharge planning and develops a resource file.
Responsibilities:
- Assists Social Work Department in implementation of discharge plan as approved by patient’s treatment team. Assists with applications to nursing homes, group homes, boarding homes, etc. Contacts outside agencies for placement, ongoing follow-up appointments as required. Arranges transportation for patients as needed by Logisticare, public transportation, or taxis. Responsible for obtaining petty cash and returning receipt to secretary. Develops relationships with outside agencies to improve process. Takes Patient out for interviews when necessary and appropriate to discharge plan. Guides families through nursing home Medicaid applications. Meets with families as directed by team.
Education or Equivalent Experience:
- Bachelor's Degree in Psychology , Sociology or a related field required.
- Experience in working with the people who have a mental illness and/or substance abuse, preferred.
