Penn Center for Connected Care (CfCC) is a centralized connected health facility for the University of Pennsylvania Health System (UPHS). The Penn CfCC is the largest academic connected health center in the nation, remotely managing thousands of UPHS patients across their care continuum. Our providers utilize state of the art technology to deliver a sophisticated layer of innovative clinical support to UPHS owned & affiliated hospitals in the PA & NJ region. Our providers are experienced clinicians who use connected health technologies as a subspecialty skill set in offering real-time clinical support on conditions that require clinical attention. In addition to the knowledge, skills, and abilities required to function as a bedside provider, providers must possess high-level skills in communication, collaboration, decision-making, systems thinking, and computer literacy, necessitated by the remote nature of the practice environment.

  • Our providers are located at a distant location on Penn Medicine Rittenhouse campus and thereby deploy software technology, hardware technology, and trended data analysis to identify evolving patient problems throughout the UPHS enterprise.
  • Our providers located on Penn Medicine Rittenhouse campus may be required at act as a telehealth presenter or facilitator to virtually connect other Penn Medicine providers to the bedside during clinical emergencies on this campus.
  • Our providers require communication with bedside clinicians via connected health technologies, therefore must be confident, composed, and responsive when interacting with patients, families, and bedside clinicians during audio-video sessions.
  • Our providers require visual acuity, keen hearing, clear distinctive speech, and manual dexterity, as well as physical tolerance to stand or sit for long periods of time at a desk in an office setting.

Minimum Requirements: Candidates must be board certified/eligible in internal medicine or family medicine.
