Summary:
- The Nurse Discharge Planner will be responsible for the coordination of a multidisciplinary plan of care ensuring the delivery of quality patient care that facilitates discharge and expected outcomes, utilizing appropriate and cost-effective strategies. They will also collaborate with the health care team to streamline patient care and identify system barriers to cost effective quality care.

- Registered Nurse - PA (Required)
- Certified Case Manager
Education or Equivalent Experience:
- Associate of Arts or Science (Required)
- RN
3+ years acute clinical experience in the designated specialty.
- Bachelor of Science Nursing
- BSN
1 + years prior acute Care discharge planning or relevant experience
1+ years leadership experience
