Penn Medicine at Home Hospice is look for a Team Manager (Nurse)
Territories: South & West Philadelphia, Main Line & some Delaware County
We are seeking a NURSE leader committed to STAFF and PATIENT experience.

Hospice experience preferred, will consider Infusion and Home Health Experience.
Schedule:
Monday-Friday
Admin on call once every 6 weeks Friday-Thursday for after-hours staff
1 winter 1 summer holiday
Summary:
- The Penn Medicine Hospice Team Manager is responsible for the financial and administrative operations of the hospice nursing team for Penn Medicine Hospice. Additionally, s/he will be responsible for the continuous development of Penn Medicine Hospice and Penn Home Palliative Care. The Team Manager will assist with the development of other Penn Medicine at Home shared services such as Home Care Central Intake and Admissions process. Along with operational responsibilities, the incumbent will develop and manage the activities of the hospice and palliative nursing teams and support the development, implementation, evaluation, and coordination of hospice nursing practices and policies across all Home Care business lines.
Responsibilities:
- Provides managerial oversite for Penn Medicine Hospice nursing teams. Functions monitored will include, but are not limited to, nursing case management, regulatory compliance, direct clinical care, new product and service development, and management of Human Resources related items for RN staff members.
- Provides active leadership and direction to nursing teams and effectively implements reengineering/workout initiatives.
- Proposes capital investments and manages them for maximum benefit to Penn Medicine Hospice.
- Partners with Finance to create operating plans and budgets to meet financial and other performance targets.
- Establishes improvement priorities and sponsors improvement activities within Penn Medicine Hospice and Penn Medicine Home Health.
- Fosters the continual integration of UPHS research and education activities into clinical and operational process.
- Ensures that all regulatory requirements are met.
- Ensures effective utilization of materials, staff, and financial resources.
- Sponsors internal and external benchmarking initiatives to establish competitive performance goals.
- Provides vision and leadership for Nursing Practice Model.
- Participates in the selection, career development, and annual performance review of their staff and peers as requested.
- Assist in improving system-wide capability by capturing and sharing best practices across the Penn Medicine at Home. Collaborates with other managers and leaders in rapid and effective implementation of these best practices in ways appropriate to each health delivery setting.
- Promote efforts to improve cost effective quality patient care in collaboration with physician and administration.
- Responsible for oversight of the infection control program and provides direction, provision and quality of infection prevention and control services as required
Credentials:
- Basic Cardiac Life Support (Required)
- Driver's License (Required)
- Registered Nurse - PA (Required)
- Car Insurance (Required)
Education or Equivalent Experience:
- Bachelor of Arts or Science (Required)
- Graduate of an NLN approved school of professional nursing
3+ years Management experience, preferably in a Home Care/Hospice/Health Care facility.
- Master of Arts or Science
- Graduate of an NLN approved school of professional nursing
