Penn Medicine at Home (PMAH) is hiring an experienced RN to service patients in our City Line territory. This is a great opportunity for an RN to truly become a partner in patient care and work at the top of their license, while impacting their community.

At PMAH, we break down our clinician treatment areas by towns/zip codes to keep commuting time to a minimum. Areas covered by this position may include: Greater Philadelphia, Philadelphia Proper and lower Montgomery County and Bucks County.

Summary:

  • Providing skilled nursing care to patients whose care is being provided by the agencys hospice or home care department specifically after business hours, evenings and nights. Functions collaboratively with the Interdisciplinary Team (IDT) by assessing patient/family needs and planning and facilitating home care, palliative care, and end-of-life care. Provides skilled nursing care and utilizes the nursing process including assessment, implementation, and evaluation in caring for the patients in urgent situations such as pain crisis or other symptom management issues.

Responsibilities:

  • Planning: Perfrming nursing assessments. Fllws established plan f care. Cmpletes the apprpriate dcumentatin required based n the assessment and implementatin f the nursing plan. Cllabrates with physicians and ther team members. Prvides cmplete handff cmmunicatin.Implementatin and Crdinatin f Services: Initiates apprpriate preventative and palliative nursing prcedures accrding t the Standards f Hspice and Palliative Nursing thrugh assessment, instructin and supprt. Crdinates care with the Triage Supervisr and Administratr n Call.Directing: Initiates patient/family instructins abut care. Maintains clear and cncise dcumentatin s that prblems, plans, actins, and gals are accurately stated and changes reflected as they ccur. Respnds t medical r emtinal crisis and/r death f a patient. Cmmunicates with the RN Case Manager/RN Team Manager regarding the patient status and fllw-up needs.Imprvement f Care: Reviews the plan f care with the RN Case Manager/Team Manager if there is current patient/family needs. Participates in perfrmance imprvement activities as requested.

Credentials:

  • Basic Cardiac Life Support (Required)
  • Car Insurance (Required)
  • Driver's License (Required)
  • RN or RN-NJ (Required)

Education or Equivalent Experience:

  • Bachelor of Science Nursing (Required)
  • BSN from an accredited school of nursing 2+ years Acute care or long term care experience as a registered nurse. 1+ years Home Care and/or Hospice experience preferred.
