  • The Licensed Practical Nurse (LPN) functions collaboratively within the team by observing the patient/family needs and facilitating care. The LPN provides direct nursing care to home health or hospice patients within the scope of practice for LPNs. The individual will follow the Plan of Care (POC) established by the RN and the Interdisciplinary Team for patients.


  • Prvides hme health r hspice services t patients thrugh regularly scheduled hme visits.Fllws established Plan f Care (PC) develped by the RN and understand the prcess fr changing the PC accrding t the patient and family needs.Perfrms apprved nursing prcedures and treatments within the LPN scpe f practice. Fllws apprpriate preventative and palliative nursing prcedures accrding t the Nurse Practice Act fr LPNs.Dcuments all interventins, patient/family respnse, and medicatin administered in the medical recrd. Maintains patient recrds s that prblems, plans, actins, and gals are accurately and clearly stated. Cmpletes patient ntes the same day that patient is seen r shift care prvided.Assists in the planning and evaluatin f the nursing care fr patients and their families.Reviews PC with the RN and RN Team Manager. Updates and revises the PC under the directin f the RN t address current patient and family needs.Participates in perfrmance imprvement activities as requested.


  • Basic Cardiac Life Support (Required)
  • BLS/CPR, as a healthcare provider as per the American Heart Association required.
  • Car Insurance (Required)
  • Car Insurance required
  • Driver's License (Required)
  • Valid Driver’s License required Reliable personal vehicle required
  • Licensed Practical Nurse (Required)
  • Pennsylvania and/or New Jersey LPN licensure depending on geographic location and scope of responsibilities required.

Education or Equivalent Experience:

  • H.S. Diploma/GED (Required)
  • H.S. Diploma/GED required. Full time experience as a Licensed Practical Nurse and 2+ years Acute care, home care, hospice care, or long-term care experience preferred.
  • Other (Required)
  • Graduate of an NLN accredited licensed practical nursing program. Full time experience as a Licensed Practical Nurse and 2+ years Acute care, home care, hospice care, or long-term care experience preferred.
