Summary:
- Responsible for contributing to the patient's treatment and treatment planning through an understanding of social, psychological and/or environmental needs related to the impact of cancer on the patient and family. Work with patient and team to resolve barriers to outpatient cancer treatment and follow-up. Professional activities will be carried out in accordance with the code of ethics established by the National Association of Social Workers.
- Position primary location is based of the Perelman Center for Advanced Medicine with some remote days.
- Member of multidisciplinary department and team
Responsibilities:
- Coordinates outpatient resources to facilitate cancer treatment and follow-up
- Collaborates and cooperates with other members of the interdisciplinary health care team.
- Identifies and attends to the psychological needs of patients and families.
- Provides counseling to patients, families and groups.
- Responsible for documentation in the medical record, statistics and reports as required.
- Facilitates legal matters, financial entitlements and complies with grants and contracts when applicable.
- Maintains relationships with community agencies and keeps up-to-date on resources.
- Uses effective interpersonal skills in all interactions
- Enhances own professional development and maintains formal and informal continuing education for self-improvement.
Credentials:
- Licensed Social Worker (State of PA)
- MSW or LSW preferred.
Education or Equivalent Experience:
- Master of Arts or Science (Required)
- Master's Degree in Social Work and 0-1 years Health Care experience in field instruction and/or employment in a health care setting required.
