Summary:

• Responsible for contributing to the patient's treatment and treatment planning through an understanding of social, psychological and/or environmental needs related to the impact of cancer on the patient and family. Work with patient and team to resolve barriers to outpatient cancer treatment and follow-up. Professional activities will be carried out in accordance with the code of ethics established by the National Association of Social Workers.
• Position primary location is based of the Perelman Center for Advanced Medicine with some remote days. 
• Member of multidisciplinary department and team

 Responsibilities:

• 1. Coordinates outpatient resources to facilitate cancer treatment and follow-up
  2. Collaborates and cooperates with other members of the interdisciplinary health care team.
  3. Identifies and attends to the psychological needs of patients and families.
  4. Provides counseling to patients, families and groups.
  5. Responsible for documentation in the medical record, statistics and reports as required.
  6. Facilitates legal matters, financial entitlements and complies with grants and contracts when applicable.
  7. Maintains relationships with community agencies and keeps up-to-date on resources.
  8. Uses effective interpersonal skills in all interactions
  9. Enhances own professional development and maintains formal and informal continuing education for self-improvement.

Credentials:

• Licensed Social Worker (State of PA)
• MSW or LSW preferred.

Education or Equivalent Experience:

• Master of Arts or Science (Required)
• Master's Degree in Social Work and 0-1 years Health Care experience in field instruction and/or employment in a health care setting required.
