Summary:
- The Social Work (SW) Lead for the Penn Partners in Care Program (PPiC) is responsible for the daily oversight of social workers who provide services to primary care practices. The SW Lead carries out this role by supervising the SW team and providing social services to individual patients. The role includes hiring, onboarding and management of the Social Workers. The Social Work Lead collaborates with the PPiC Clinical and Administrative leads along with leaders from the hospitals, home care, and post-acute teams to assist in patient delivery. The SW Lead participates in coordinating and improving care management workflows and identifies opportunities to further develop or improve services.
Responsibilities:
- Provides professional guidance, oversight, and support for the PPiC Social Workers
- Recommends and implements changes to develop the role of the PPiC Social Workers
- Hires, and on-boards Social Workers, assists in maintaining an orientation and training program for the Social Work role
- Provides daily oversight of staffing, schedules, and payroll for the Social Workers
- Manages referral pools and delegates daily assignments to the Team
- Prepares and delivers performance evaluations for the Social Workers
- Maintains support for an individual panel of Social Work referrals in addition to supervisory responsibilities up to 50% of
- total hours worked
- Collaborates with the Clinical Leads & Program Director to develop and improve workflows and processes to support the
- Social Work role
- Interacts with hospital, home care and post-acute program managers to promote effective communication channels to
- support patients
- Interacts with Practice Managers across the regions to assure coordinated and efficient delivery of care management
- services
- Participates in committees or workgroups to support the improvement and development of the care management program
- Runs performance and volume reports for the team and reviews and analyzes program data to assist in program evaluation
- and team productivity
- Uses quality improvement strategies to identify opportunities to improve services
- Participates in payer initiatives that pertain to social services
- Represents PPiC in community collaborative meetings for social work with other Philadelphia health systems
- Coordinates with Penn-supported social resource volunteer groups to supplement PPiC social services to patients
- Participates in PPiC program meetings
- Represents PPiC with the social platform vendor (Findhelp) by attending relevant meetings
- Provides staff support by participation in the PPiC staff meetings, SW meetings and one to one staff meetings
- Performs duties in accordance with Penn Medicine and entity values, policies, and procedures
- Other duties as assigned to support the unit, department, entity, and health system organization
Credentials:
- Licensed Social Worker required
- Dual PA & NJ License obtained within 6 months of hire required
Education or Equivalent Experience:
- Master's Degree Social Work required
- 3+ years Relevant healthcare experience required
- 1+ years Prior supervisory experience preferred
