Summary:

  • A registered pharmacist who performs specific job responsibilities in accordance with the New Jersey State Board of Pharmacy, Federal and State Regulatory Agencies, and JCAHO Standards, ASHP practice guidelines and organizational policies and procedures.

Responsibilities:

  • Manages the daily operation of the Out-patient Pharmacy area. Provides medications and pharmacy services to defined outpatients (i.e., clinic, employees, hospice and discharged inpatients). Reviews pertinent patient clinical information to ensure medication prescription is appropriate. Confers with physicians and other healthcare providers to resolve discrepancies and problems with medication prescriptions regarding the appropriateness, dosage, drug interactions, thoroughness and formulary status. Performs all necessary documentation and coordination of services required. Provides drug information to physicians and other healthcare professionals as requested. Performs all dispensing functions of a registered pharmacist. Reviews written prescriptions, and processes prescriptions utilizing the electronic database processing system. Provides medications in quantities as per applicable law and organizational policies. Verifies medications filled by pharmacy technicians prior to dispensing. Provides patient medication information and counseling if appropriate. Files and maintains completed prescriptions as per applicable law.

Credentials:

  • Pharmacist (Required)
