Department: Cardiac Surgery

Location: Hospital of the University of Pennsylvania- 3400 Spruce St

Hours: Day shift

  • The Licensed Practical Nurse (LPN) supports providers (APPs, physicians) and supportive staff with the Heart Valve Center patient population. The LPN will ensure compliance with clinical policies, protocols, and national registries. The LPN will engage in patient interactions including but not limited to phone triage, abstract incoming consults, perform patient frailty assessments, and provide patient and family education.

    The LPN will also engage in non-patient interaction tasks including but not limited to preparing procedural cases plans on a weekly basis, making professional judgments and appropriately reporting clinical data to internal and external customers. Requires a detail-oriented candidate with strong organizational, time management, interpersonal and communication skills. Must be flexible to respond to requests and meet deadlines. Able to work collaboratively with team members as well as independently. Must be proficient in Microsoft Office, EHR. Prior cardiology and/or cardiac surgery experience preferred.

Responsibilities:



  • Patient/Customer • Patient/family teaching and education • 100% HIPAA compliance with patient data • Abstract and communicate details of new consultations to providers • Ensure diagnostic imaging is available prior to new consult appointment • Perform frailty assessments as directed • Ensure proper EHR documentation per department guidelines, mandatory state/federal guidelines • Telephone/Virtual triage of patient/family questions • Data Collection • Monitor databases for accuracy and compliance and retrieve missing data as needed • Monthly, Quarterly and fiscal year end reporting as requested • Maintain protocols and procedures for Quality and Safety Outcomes • Maintain ongoing documentation and compliance with Continuing Medical/Nursing Education Office requirements for weekly continuing education • Performs duties in accordance with Penn Medicine and entity values, policies, and procedures • Other duties as assigned to support the unit, department, entity, and health system organization

Education or Equivalent Experience:

  • H.S. Diploma/GED (Required)
  • 1+ years Outpatient experience preferred
  • Other (Required)
  • Practice Nursing School
