Position Summary:
The Oncology Nurse Navigator (ONN) provides care coordination to cancer patients with the goal of overcoming potential or realized barriers to care, providing education and resources to facilitate timely delivery of services and psychosocial care, and improving patient outcomes. The ONN collaborates with the multidisciplinary team as an advocate, educator, and counselor for UMCPP oncology inpatients and outpatients across the continuum of care and serves as a liaison throughout the facility and in the community regarding services provided for the oncology patient population.
Essential Job Duties and Responsibilities:
1. Identifies potential and realized barriers to care (e.g. transportation, child and/or elder care, housing, language,
culture, literacy, role disparity, psychosocial, employment, financial, insurance) facilitating referrals as appropriate to
mitigate barriers.
2. Provides and/or reinforces education to patients, families, and caregivers about diagnosis, treatment options, side
effect management, and post treatment care/survivorship. Collaborates with the survivorship coordinator to provide
survivorship care.
3. Tracks metrics and patient outcomes to document and evaluate outcomes of the navigation program.
4. Participates in coordination of the plan of care with the multidisciplinary team promoting timely follow up on treatment
and supportive care recommendations. Acts as a liaison and advocate between patients, families, caregivers, and
providers to optimize patient outcomes.
5. Collaborates with the cancer committee and UMCPP administration to ensure compliance of standard 3.1 American College of Surgeons Commissions on Cancer (ACoS CoC) is fulfilled and maintained.

Education
Bachelor’s Degree required
Master’s Degree (preferred)
Related Work Experience
Minimum of 5-8 of nursing experience required
Licensure, Certification, or Registration Requirements
Current license to practice nursing in State of NJ required
OCN certification
