Hall Mercer at Pennsylvania Hospital is searching for a full-time outreach worker to work Monday through Friday 6:00 am - 2:00 pm.
Summary:
- The role of a Homeless Outreach Worker is to provide general community outreach and focused engagement for people experiencing homelessness on the streets of Philadelphia. Outreach Workers collaborate with the Department of Behavioral Health and Intellectual Disabilities Services (DBHIDS), Outreach Coordination Center (OCC), other Outreach providers, the Office of Supportive Housing (OSH), and other community and private providers. Outreach workers offer emergency housing, medical, and behavioral health treatment options, and assistance to help individuals meet their immediate survival needs. Outreach workers build reliable relationships with the homeless community.
Responsibilities:
- Adheres to Performance Standards and professional development as mandated by the Department of Behavioral Health (DBHIDS) and Hall-Mercer Maintains a 20 person focus list (full time employee) or 10 person focus list (part time) independently with minimal supervisory or "third-party” intervention. Enters daily contacts by the end of shift into the Web Focus System. Responds to Response Calls in a timely manner, provides all available supports and ensures appropriate follow-up activities occur with the OCC. Continuously travels throughout shift in team’s respective zones to engage homeless individuals. Utilizes program and facility resources appropriately and within guidelines. Meets standard of 17 hours of staff development, experiential learning, professional training per year. Provides Outreach and Engagement Services as mandated by the Department of Behavioral Health Offer housing options for those who are interested. Transport those who are interested to housing, medical, psychiatric, or other resources. Call in a 302 (involuntary psychiatric hospitalization) when appropriate.
Credentials:
- Basic Cardiac Life Support (Required)
- Driver's License (Required)
- Valid driver's license
Education or Equivalent Experience:
- Associate of Arts or Science (Required)
- And 0-1 years experience in creative writing for business
Education Specialization: or Bachelors degree in communications or similar field
