Summary:
- Responsible for discharge planning and contributing to the patients treatment plan through and understanding of social, psychological and/or environmental needs related to the impact of illness and need for hospitalization. Professional activities will be carried out in accordance with the code of ethics established by the National Association of Social Workers.

- Crdinates the discharge planning prcess t ensure a safe and timely transitin frm hspital t hme, cmmunity, r institutinal setting.Cllabrates and cperates with ther members f the interdisciplinary health care team t ensure cntinuity f care.Identifies and attends t the psychlgical needs f patients and families.Prvides cunseling t patients, families and grups.Respnsible fr dcumentatin in the medical recrd, statistics and reprts as required.Facilitates legal matters, financial entitlements and cmplies with grants and cntracts when applicable.Maintains relatinships with cmmunity agencies and keeps up-t-date n resurces.Uses effective interpersnal skills in all interactinsEnhances wn prfessinal develpment and maintains frmal and infrmal cntinuing educatin fr self imprvement.
Credentials:
- Licensed Social Worker
- MSW or LSW preferred.
Education or Equivalent Experience:
- Master of Arts or Science (Required)
- Master's Degree in Social Work and 0-1 years Health Care experience in field instruction and/or employment in a health care setting required.
