Inpatient Psychiatric Consult Team Social Worker Job Posting

Position Description:

Responsible for providing and documenting individualized, person-centered psychosocial assessment, behavioral health care planning, post-discharge substance use inpatient and outpatient community support, and ongoing emotional support for individuals with mental health concerns and their families within an acute care hospital setting. As part of an interdisciplinary consultative psychiatric team, the social worker collaborates with members of their own team plus interdisciplinary members of various other medical teams caring for the patient during their hospitalization. The psychiatric consult team social worker helps to ensure continuity of care and a behavioral health care plan and discharge plan that are both in alignment with the patient and family goals of care. The psychiatric consult team social worker also assists the team in providing ongoing education and support to hospital staff across disciplines. Professional activities will be carried out in accordance with the code of ethics established by the National Association of Social Workers.

Position Accountabilities:

· Work collaboratively with an interdisciplinary psychiatric consultation service team of physicians, advance practice providers, counselors, and other learners to provide supportive care to patients with serious illnesses and their families.

· Effectively communicate and coordinate with interdisciplinary members of various other medical teams caring for patients in order to ensure continuity of care and to help facilitate complex discharge planning with post-hospitalization behavioral health care needs.

· Complete psychosocial assessments with focus on patient and families level of coping with psychiatric illness and co-occurring substance use/abuse and/or complex medical conditions.

· Assess and treat individuals with mental, emotional, and/or substance abuse problems, including abuse of alcohol, tobacco, and/or other substances. Potential interventions include: brief supportive therapy, motivational interviewing, engaging patients in non-pharmacological techniques for symptom management (e.g. mindfulness, deep breathing, etc), crisis intervention, case management, client advocacy, prevention, and education.

· Provide ongoing emotional support and psychoeducation to patients and families in coping with psychiatric diagnoses and illness journey.

· Assist patients in coping with new medical diagnoses, mental illness, substance use, poverty, unemployment, or physical abuse.

· Document in electronic medical record in a timely manner utilizing appropriate psychosocial assessment and consultative psychiatric team progress notes.

· Maintain relationships with behavioral health inpatient units, drug/alcohol counseling and support centers, and keep up-to-date on relative resources and laws regarding behavioral health.

· Plan, facilitate, and participate in relevant programs for interdisciplinary staff both within the hospital and across the health system.

· Participate in Case Management & Social Work Department meetings and join department committees as time allows.

· Use effective interpersonal skills in all interactions.

· Participant in ongoing professional development and maintain formal and informal continuing education for self-improvement.

· Supervise and mentor BSW and MSW student interns.

· Perform duties in accordance with Penn Medicine and entity values, policies, and procedures.

· Other duties as assigned to support the inpatient psychiatric consult team, the Case Management & Social Work department, Pennsylvania Hospital, and the University of Pennsylvania Health System.

Position Minimum Requirements:

· Master’s Degree in Social Work (MSW)

· Two years of post-Masters experience in a healthcare and/or behavioral healthcare setting

· At least one year of related psychiatric and/or behavioral health care experience preferred

· Familiarity with Philadelphia behavioral health systems strongly preferred

    · Pennsylvania Social Work License (LSW or LCSW) preferred
