Pennsylvania Hospital - Hall Mercer is searching for a Case Manager to join our Intensive Service department. The hours would be Monday through Friday 9:00 am- 5:00 pm.
Summary:
- This Case Manager will be working with adults who have an intellectual disability. The Case Manager will help with locating emergency placement as well as getting them registered with IDS if needed. Providing assistance to community-based adults with intellectual disabilities (or dually diagnosed MH/ID) who are currently or soon to be homeless, or currently living in abusive or neglectful situations. This Case Manager will also assesses consumers’ emergency needs and assists in obtaining housing and/or services through the intellectual disabilities system.
Responsibilities:
- Assessment of emergency housing need
- Assessment of individuals to ensure that they meet Personal Care Home criteria
- Referrals to compatible Personal Care Homes
- Initiates registration with Intellectual Disability Services (IDS) by compiling a social history and other supporting documents and arranging for a psychological evaluation through the county psychologist
- Provides interim case management until a permanent supports coordinator is assigned
- Maintains 24-hour emergency hotline for IDS, the Supports Coordination Organizations and all non-registered individuals with intellectual disabilities living in Philadelphia County
Credentials:
- Bachelor of Arts or Science And 2+ years MH/MR Experience Education Specialization
OR
- H.S. Diploma/GED And 4+ years MH/MR Experience Education Specialization: At Least 15 College Credits in Human Services
