General Summary Statement

Primary role is that of a patient educator. Position includes instruction of diabetes self-management training curricula, providing one-on-one diabetes and/or nutrition assessment and training. Position requires thorough completion of all materials used in documentation in Penn Chart. A diabetes educator provides services with respect for the uniqueness, dignity and autonomy of each individual as stated in the ADCES Scope of Practice and Standards of Practice for Diabetes Educators.

Essential Duties and Responsibilities

Diabetes Education

  1. Utilizes evidence-based guidelines in individual and group teaching situations.
  2. Applies knowledge of principles of teaching and learning to provide education that is age and culturally appropriate.
  3. Demonstrates critical thinking skills in assessment of patients and the development of care plans.
  4. Completes all documentation in EPIC related to patient documentation and physician follow-up in a thorough, clinically appropriate and timely manner.
  5. Communicates via EPIC with physicians and other healthcare team members as required or needed according to department policies.
  6. Demonstrates proficiency with operation and download of blood glucose meters, CGM (continuous glucose monitoring devices) both personal and professional and insulin pumps.
  7. Attends and actively participates in staff meetings.
  8. Assists in CQI activities such as chart reviews and projects as assigned.
  9. Serves as role model or mentor for other health professionals or students. Supervises student interns as assigned.
  10. Assists with community outreach needs as assigned.
  11. Assists in the development and maintenance of patient education materials and curriculum as requested.
  12. Provide caregiver training Med Techs in skilled facilities when scheduled.
  13. Provides diabetes education for inpatients as requested for patients who have consultation requests in EPIC.

Professional Development

  1. Maintains memberships in related professional organizations to demonstrate a commitment to professional growth and to stay up to date in the profession.
  2. Completes at least 15 hours annually of continuing education in diabetes care/management.
  3. RD Diabetes Educators will also maintain continuing education requirements for Commission on Dietetic Registration learning portfolio.

Education Training and Experience

Bachelor of Science in Nutrition, Nursing or related health care field required, Master’s degree desirable. Three years or more experience in diabetes education and outpatient counseling desirable.

Licensure/Certification

Registered Dietitian or Registered Nurse

PA State Licensure

CDCES or obtain when 2000 hours has been met
