The Oncology Infusion Pharmacist is responsible and accountable for the provision of safe, effective and prompt medication therapy in the ambulatory infusion centers. The Oncology Infusion Pharmacist collaborates with providers and nurses to ensure safe and effective chemotherapy and adjunctive medications for ambulatory oncology and non-oncology infusion patients. The Oncology Infusion Pharmacist is responsible for overseeing both hazardous and non-hazardous sterile compounding and ensuring compliance with regulatory requirements. The Oncology Infusion Pharmacist collaborates with physicians, advanced practice providers, nurses, and other members of the healthcare team to deliver optimal medication therapy to patients with a broad range of disease states. Pharmaceutical care services include, but are not limited to, assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. The pharmacist provides medication and practice-related education/training by serving as a preceptor for doctor of pharmacy students and PGY1 and PGY2 pharmacy residents.
