Summary:

  • The One Ambulatory Pharmacy Clinical Pharmacist is responsible for ensuring Penn Medicine patients requiring infusion, injectable, or specialty medications are treated based on geographic location, patient preference, and payer direction.  The Clinical Pharmacist will maintain current knowledge of managed care shifts, payer strategies, medical policies, and PBMs.  They will support Penn Medicine providers and Clinical Pharmacy Specialists by removing administrative burdens and performing order entry, appeals, and patient reassessments.  They will adhere to all local, state, and federal regulatory guidelines in accordance with Penn Medicine's values, policies, and procedures.  They will be responsible for completing other duties, as assigned, to support the department, entity, and health system.

Responsibilities:

  • Provides clinical decisions regarding patient-specific pharmaceutical care, including assessment of the patient's general pharmaceutical needs, review of the patient's medication profile, evaluation for duplicate therapies, evaluation for drug interactions, and development of patient database and documentation in the medical record.
  • Utilizes available information (formularies, medical policies, etc.) to determine the appropriate site of service for patients.
  • Acts as part of the care team, supporting Physicians, Nurses, and APPs by pending prescriptions or Beacon Treatment plans as necessary, drafting LMN, and supporting peer-to-peer conversations.
  • Effectively communicates and documents actions with Physicians, Nurses, APPs, and patients in the EMR.
  • Continual update and maintenance of required documentation by State and Federal government as well as regulatory agencies (TJC, URAC, etc.).

Credentials:

  • Pharmacist License in Pennsylvania (Required)

Education or Equivalent Experience:

  • Bachelor of Pharmacy  (Required)
  • 1+ years' experience with PBM, payer relations, infusion, specialty or ambulatory care pharmacy
  • Other
  • PGY1 Residency
