Summary:

  • Manages the clinical and production pharmacy services provided to assigned patients. This position is responsible for the checking of filled orders and supplies, performing sterile compounding as needed, and may oversee the work of pharmacy technicians and other pharmacists.

Schedule requirements: 

  • Hours: 12:30pm-9pm
  • 1 Weekend every 4-8 weeks
  • 1 Holiday/ year 

Responsibilities:

  • Provides clinical decisions regarding the patient-specific pharmaceutical care including: assessment of the patient(s) general pharmaceutical needs, review of patients’ medication profile, evaluation for duplicate therapies, evaluation for drug interactions (drug-drug, drug-nutrient, drug-disease), and development of patient database and documentation in medical record.Accepts physician prescription orders and evaluates the appropriateness of these orders for dosage, drug choice, potential drug interactions, route of administration, stability, compatibility, and mode of administration (i.e. infusion device).Participates in the development of a multi-disciplinary Plan of Care, establishment of goals and objectives of prescribed therapy in conjunction with physician, and recommendations for the most efficient and effective dose administration.Monitors patient(s) response to treatment by subjective and objective assessment. Monitors laboratory tests when necessary and obtains and discusses the results with nurses and physicians of the patient. Provides recommendations to improve/adjust therapy based on lab evaluations. Provides patient specific drug information to patient/caregiver, prescribing physicians, contracted nurse agencies, and other Penn Home IT clinicians.Communicates effectively and documents actions with physicians, nurses, and patients in the software database. Ensures that the patient database is complete for relevant information related to medical status and therapy. Updates and maintains required documentation by State and Federal government as well as JCAHO (i.e. prescription logs, patient medication profiles, patient charts, narcotic receipts, state pharmacy, and narcotic permit).Dispenses medication accurately and dispenses ancillary infusion supplies appropriate for diagnosis and therapy prescribed. Supervises and inspects medication preparation of the pharmacy technicians.Assists the physician nurse, and patient/caregiver with clinical and technical pharmacy support. Communicates daily with nursing, pharmacy technicians, physicians, patient service representatives, materials management, and distribution to address issues to provide continual positive patient outcomes.

Credentials:

  • Pharmacist (Required)
  • Must be a Licensed Pharmacist in the State of Pennsylvania (required)

Education or Equivalent Experience:

  • Bachelor of Pharmacy (Required)
  • Education Specialization: Science in Pharmacy from an accredited College of Pharmacy Equivalent Experience: •And 0-1 years Less than 1 year of pharmacist experience required
  • Other
  • Education Specialization: Doctor of Pharmacy Degree from an accredited College of Pharmacy. Equivalent Experience: •And 0-1 years Less than 1 year of pharmacist experience required
