Working for this leading academic medical center means collaboration with top clinical, technical and business professionals across all disciplines.Chester County Hospital is seeking an Advanced Practice Provider to join the inpatient Palliative Care Team. Palliative Care specialists provide pain and symptom management, psychosocial support, and advance care planning for patients and families facing serious illness. The inpatient palliative care clinician is responsible for working as a consultant throughout the hospital, visiting directly with patients and families and offering recommendations to the primary healthcare team.
Our interdisciplinary team collaborates to provide expert patient- and family-centered palliative care focused on the patient’s psychosocial, spiritual, and medical needs. Consultations cover all acute care settings, including general medical-surgical floors, specialty units, and critical care areas.
The NP/PA will provide expert symptom management and guide crucial discussions with patients and families to promote a personalized, comprehensive care plan that aligns with patient goals. The NP/PA will also participate in daily inter-professional palliative care team rounds to support the interdisciplinary practice model and to facilitate team communication and collaboration.
Minimum Requirements:
• Master’s prepared Nurse Practitioner with Acute Care certification or Master's prepared Physician Assistant
• Current Pennsylvania license (or eligible to obtain)
• A minimum of 2-3 years of experience as an Advanced Practice Provider in the acute care setting is strongly preferred
· Training/experience in palliative care, hospice, or geriatrics is preferred but not required
· Candidates should possess strong communication skills and the ability to work independently with a wide variety of medical and surgical services
Additional Information: Monday - Friday 8:30am - 5:00pm - No weekends or on-call requirements.
