The Team Lead for Specialty Programs plays a pivotal role in ensuring the seamless implementation of specialty program protocols and delivering high-quality patient care for those enrolled in our PMAH Specialty Programs, such as PATH (Practical Alternative to Hospitalization). This dynamic position acts as a central liaison among various departments—including Emergency and Inpatient Departments, Penn Medicine Home Health, and Specialty Programs—to coordinate patient flow, optimize resources, and support efficient clinical workflows.

The Team Lead will oversee the operational management of clinical protocols, ensuring the right resources are in place to address urgent patient needs. This role will also collaborate with Central Intake, Scheduling, and care teams to ensure patients receive timely, high-quality care at home.

The ideal candidate will possess strong leadership skills, with experience managing teams and coordinating care across multiple departments. They should demonstrate excellent communication and interpersonal abilities, enabling effective collaboration with both clinical and non-clinical teams. Additionally, the candidate will have a proven ability to manage operational workflows, optimize resource allocation, and support clinical staff in delivering high-quality patient care. Familiarity with Penn Medicine systems and protocols is a plus.

Shift: 11:00 AM - 9:00 PM with every third weekend requirement of 9:00 AM -7:00 PM.

Please note you have to live or be willing to relocate to the state of PA, NJ, or DE.

Responsibilities:

  • Serve as the primary liaison between inpatient, ambulatory, and home health teams, facilitating smooth communication and collaboration.
  • Coordinate patient flow and optimize scheduling with Central Intake and Scheduling Departments to ensure efficient patient care.
  • Oversee the operational execution of specialty program protocols, including resource allocation and workflow optimization.
  • Support in-home clinical staff, guiding them in interpreting and implementing specialty program protocols and serving as a consultative resource.
  • Work closely with the Team Manager to manage staffing and resources, ensuring that patient needs are met promptly and efficiently.
  • Act as a key resource for nurses and other personnel, supporting clinical practice and the implementation of high-quality specialty care in the home.
  • Address unplanned or urgent patient needs by aligning services and coordinating with courier or other support services to ensure seamless care delivery.

Credentials:

  • Registered Nurse (Required)

Education or Equivalent Experience:

  • Bachelor of Arts or Science (Required)
