Summary:

  • The Clinical Pharmacist is responsible for conducting medication reconciliation and clinical assessments for specialty pharmacy patients to promote positive patient outcomes. The role involves supporting centralized and decentralized medication-use systems, and collaborating with physicians, advanced practice providers, nurses, and other healthcare team members to deliver optimal medication therapy for a wide range of disease states. Pharmaceutical care services encompass assessing patient needs, tailoring drug therapy and education to age and disease-specific characteristics, adjusting care based on patient response, and providing clinical interventions to detect, mitigate, and prevent adverse medication events. Responsibilities will also include expanding patient outreach and access through outbound calls educating patients about Penn Medicine Specialty Pharmacy integrated care model.

Responsibilities:

  • Engage with patients onsite within clinics or virtually, perform medication reconciliation on each patient, and review Penn Specialty Pharmacy’s integrated care model
  • Review patients' electronic medical records and chart notes to understand their conditions and medication regimens prior to calls. Perform necessary clinical interventions, adhering to the most up-to-date treatment guidelines
  • Act as liaison between providers and advocate for patients to ensure optimal care and treatment, promoting positive patient outcomes
  • Conduct patient outreach through outbound calls to improve access to care and reduce time to therapy
  • Contribute to, participate in, and lead multidisciplinary quality improvement initiatives and clinical research within the pharmacy and/or other institutional departments to improve patient care
  • Supervise and direct the activities and growth of other pharmacists, residents, technicians, interns, and students
  • Perform and support with other clinical duties as needed (e.g., provide concise, applicable and timely responses to drug information requests from health care providers and patients)
  • Verify, prepare and dispense medications using appropriate techniques to ensure positive outcomes of drug therapies and compliance with regulatory requirements and the organization’s policies and procedures. Ensure adherence to medication-use policies and guidelines

Credentials:

  • Pharmacist (Required)
  • Current RPh License in Pennsylvania or eligible for licensure in PA.

Education or Equivalent Experience:

  • Bachelor of Arts or Science (Required)
