Working for this leading academic medical center means collaboration with top clinical, technical and business professionals across all disciplines.Function: Acts as primary contact for homeless shelter / mental health residential providers with Hall-Mercer. Coordinates care as a member of a multi-disciplinary treatment team providing outreach and psychiatric services to a chronically mentally ill and dually diagnosed homeless Adult consumer population.
Duties:
1. Acts as primary point of contact for homeless shelter / mental health residential providers with Hall-Mercer.
2. Responsible for providing, in consultation with staff physician, mental health evaluations at Hall-Mercer and community homeless service agencies.
3. Coordinates, provides and facilitates connection of consumers to a full range of outreach services, engagement activities, required case management, clinical outpatient services, substance abuse services, residential care and linkage to other community supports.
4. Maintains an active caseload of outreach / engagement consumers throughout the affiliated homeless shelter / mental health residential sites as well as maintains active caseload of case management consumers attending outpatient medication management clinical care at Hall-Mercer.
5. Collects clinical information, prepares and maintains appropriate paperwork, registers to appropriate source and level of treatment & implements treatment plans for consumers who consent to outpatient care and / or services at Hall-Mercer.
6. Maintains records of contacts within the homeless shelter / mental health residential sites, documents all services provided to consumers, maintains as current outpatient clinical charting in accordance with Homeless Outreach Service specific, Hall-Mercer and other agency protocols & completes staff development training requirements.
7. Other duties as assigned.
