Summary:
The palliative care social worker is an essential member of the palliative care team. Working alongside physicians, APP's and chaplains to provide patients and families with consistent and compassionate care. The palliative care social worker conducts psychosocial assessments, provides supportive counseling, coordinates and facilitates goals of care discussions, and streamlines care across the care teams for patients and families facing serious or life-threatening illness.
The palliative care social worker is responsible for providing and documenting individualized, person-centered psychosocial assessment, advance care planning, discharge planning and on-going emotional support for individuals with serious illnesses and their families within an acute care hospital setting. As part of the interdisciplinary consultative palliative care team the social worker in addition to this team will collaborate with members of various other medical teams caring for patients during their hospitalization. The palliative care social worker is responsible for ensuring continuity of care and a medical care plan and discharge plan that are both in alignment with the patient and family goals of care.
