Care Connections is a short-term program integrating medical and care management services. Our team-based approach helps connect the dots for patients with complex challenges. Our team is looking for a board-eligible or board-certified Family Medicine, Internal Medicine, or Geriatrics physician to join our team. Our team consists of patient care navigator, nurse case manager, clinical pharmacist, physicians, advanced practice providers, social workers, clinical service specialists, and an in house chaplain. 

Care Connections sees patients in inpatient, outpatient, and home care settings. Our holistic approach to patient care allows us to focus on patient well-being and provider satisfaction. Our physicians enjoy a Monday through Friday dayshift schedule with minimal call shared within the group. The ideal candidate will be passionate about serving the underserved population of Lancaster and comfortable with handling complex cases.

Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

• Consults with patients to determine the appropriate course of treatment.
• Recommends and orders tests to determine extent of illness or help diagnose condition.
• Monitors lab results, medication adherence and medication toxicities.
• Prescribe or administer treatment, therapy, medication, vaccinations, and other medical care to treat or prevent illness, disease, or injury.
• Explain and discuss procedures, test results, or prescribed treatment with the patient.
• Coordinate work with nurses, social workers, rehabilitation therapists, pharmacists, psychologists, and other health care providers.
• Refers patients to medical specialists when necessary.

Minimum Qualifications:

• Medical Doctor (MD), Doctor of Osteopathy (DO), issued by the Accreditation Council on Medical Education. 
• Certified by the American Board of Family Medicine/Internal Medicine or Board eligible.
• Successful completion of Family Medicine or Internal Medicine residency.
