Clinical Pharmacist Specialist- Emergency Medicine- Full Time- Pennsylvania Hospital

Summary:

• This position is responsible for provision of pharmaceutical care for patients under the care of the Emergency Department. Activities include participation in interdisciplinary care planning; patient education; evaluate, monitor and modify drug therapy; provide recommendations to optimize patient outcomes; minimize safety issues; ensure cost effectiveness; and provide drug information. The Clinical Pharmacist Specialist participates in the development of programs, policies, and guidelines that support the implementation of evidence-based standards of practice and fulfill requirements of UPHS, Pennsylvania Hospital, PAH Pharmacy Department, TJC, and other regulatory agencies. The Clinical Pharmacist Specialist contributes to the overall clinical management initiatives of the Department of Pharmacy Services through the provision of Pharmacy in-services; quality initiative implementation, data collection, and analysis; contributing to the detection and reporting of adverse drug events; and participating as a regular or ad hoc member of hospital committees.

Credentials:

• Doctor of Pharmacy Degree from an accredited College of Pharmacy
• Completion of a Pharmacy Practice Residency (PGY-1) or equivalent experience
• Board certification or certification eligibility in pharmacotherapy management
  
•  Experience precepting students and residents preferred
• Completion of a PGY-2 residency or equivalent experience preferred
  
• Current Registered Pharmacist license in the Commonwealth of Pennsylvania
