Pennsylvania Hospital is seeking a full time Clinical Pharmacist Specialist to join our ICN team !
The Intensive Care Nursery is a 50 bed level 3 intensive care unit that specializes in the care of premature and critically ill infants. As part of the largest delivery service in Philadelphia, this busy unit cares for infants who require both long term and transitional care. The Intensive Care Nursery is a collaborative environment that uses the skill and expertise from a variety of disciplines to provide the highest level of care in a family centered environment. The unit participates in numerous research studies aimed at elevating neonatal care.
This position is responsible for provision of pharmaceutical care for patients in the Intensive Care Nursery. Activities include participation in interdisciplinary patient rounds; patient and caregiver education; evaluate, monitor and modify drug therapy; provide recommendations to optimize patient outcomes; minimize safety issues; ensure cost effectiveness; support the investigational product portions of the clinical trials conducted within the Women’s Health Division; and provide drug information. The Clinical Pharmacist Specialist participates in the development of programs, policies, and guidelines that support the implementation of evidence-based standards of practice and fulfill requirements of UPHS, Pennsylvania Hospital, PAH Pharmacy Department, TJC, and other regulatory agencies. The Clinical Pharmacist Specialist contributes to the overall clinical management initiatives of the Department of Pharmacy Services through the provision of Pharmacy in-services; quality initiative implementation, data collection, and analysis; contributing to the detection and reporting of adverse drug events; and participating as a regular or ad hoc member of hospital committees.
Board certification or certification eligibility in pharmacotherapy management
Completion of a Pharmacy Practice Residency (PGY-1) or equivalent experience
Experience precepting students and residents preferred
Completion of a PGY-2 residency or equivalent experience preferred
Current Registered Pharmacist license in the Commonwealth of Pennsylvania
