Summary:

  • Responsible for discharge planning and contributing to the patient's treatment plan through and understanding of social, psychological and/or environmental needs related to the impact of illness and need for hospitalization. Professional activities will be carried out in accordance with the code of ethics established by the National Association of Social Workers.

Unit Description:

The Emergency Department (ED) at Pennsylvania Hospital, located in the historic district of downtown Philadelphia, is a 29 bed unit that sees over 36,000 visits annually. Although not a Level One Trauma Center, the ED treats a diverse patient population in virtually all specialties.

The ED focuses on performance improvement and evidenced-based practice to provide the best possible care. In 2018, the ED was awarded the Emergency Nurses Association (ENA) Lantern Award. This award is given to only one percent of emergency departments nationally and recognizes exceptional practice and innovative performance in the core areas of leadership, practice, education, advocacy, and research. The Pennsylvania Hospital Emergency Department is the first hospital in Philadelphia to earn such a prestigious designation.

Staff at all levels actively participate in unit councils, shared governance and various committees, always striving to provide the most compassionate, quality care possible. From our care delivery model of modified primary care, through our professional practice model of Relationship-Based Care (RBC), the department is proud of our excellent work. Whether it involves a family crisis or a cut finger, our staff always stays focused on the patients and their families’ immediate needs. The ED contributed several sources of evidence to the Magnet document, assisting the hospital in becoming Magnet-designated by the American Nurses Credentialing Center (ANCC) in 2018.

Responsibilities:

  • Coordinates the discharge planning process to ensure a safe and timely transition from hospital to home, community, or institutional setting.
  • Collaborates and cooperates with other members of the interdisciplinary health care team to ensure continuity of care.
  • Identifies and attends to the psychological needs of patients and families.
  • Provides counseling to patients, families and groups.
  • Responsible for documentation in the medical record, statistics and reports as required.
  • Facilitates legal matters, financial entitlements and complies with grants and contracts when applicable.
  • Maintains relationships with community agencies and keeps up-to-date on resources.
  • Uses effective interpersonal skills in all interactions
  • Enhances own professional development and maintains formal and informal continuing education for self improvement.

Credentials:

  • Licensed Social Worker
  • MSW or LSW preferred.

Education or Equivalent Experience:

  • Master of Arts or Science (Required)
  • Master's Degree in Social Work and 0-1 years Health Care experience in field instruction and/or employment in a health care setting required.
