The Patient Navigator functions as a resource to the Penn Transplant Living Donor Program for patient navigation and coordination of services. The Navigator is responsible for ensuring that Living Donor patients receive quality and comprehensive coordinated services as directed by the Living Donor transplant team. The Navigator will coordinate care throughout the continuum. The scope of this work includes: facilitating scheduling for patients, medical records, faxing/emailing, confirming contact information, providing directions and appointment related specifics, Epic documentation data collection and other duties as assigned. Additional projects/assignments related to Patient and Families Services as assigned by Manager or Director.

Responsibilities:

  • Systematically and continually perform the functions of assessing, planning, implementing and evaluating the care of the Living Donor Transplant patient
  • Support the patient/family and assist in coordination of care for the patient and family
  • Function in an organized and time-conscious manner. The Navigator partners with patients, families, the Living Donor team and related departments to provide well-coordinated, timely, compassionate and multidisciplinary care. The Associate Navigator communicates with all members of the health care team as appropriate about the patient/family needs and concerns
  • Work across health care disciplines to assist in coordinating patient tests, procedures, appointments and treatments and assist patients in overcoming barriers to care including transportation, financial counseling, referrals, etc.
  • Refer patients for supportive care services including nutritionists, social workers, counselors and financial counselors in coordination with Living Donor Transplant team
  • Monitor patients throughout their care in the system
  • Communicate in-person or via phone, letters, email with patients, visitors, staff and physicians
  • Identify potential problem areas such as service breakdowns, communication issues, scheduling difficulties, etc and contribute to solutions and improvements
  • Document all work performed in the electronic health record, and/or other data collection platform

Credentials:

  • Basic Cardiac Life Support (Required)
  • Medical Assistant
  • MA Certification

Education or Equivalent Experience:

  • H.S. Diploma/GED (Required)
  • 1+ years Clinical experience in a hospital/health care setting with patients, families, physicians, staff and administration is preferred
  • Other (Required)
  • Completion of an accredited Medical Assistant program (or higher level equivalent). 7+ years experience working as an MA or other patient facing health care experience may be considered in lieu of MA program completion.
